Assumption of Risk
I understand that participation in Brazilian Jiu-Jitsu, submission grappling, wrestling, and related
martial arts training at Not Bad BJJ ("the Academy") involves the risk of serious injury, including but
not limited to bruising, cuts, sprains, joint injury, concussion, dental injury, dislocation, broken
bones, and in rare cases catastrophic injury. I voluntarily accept and assume these risks.
Skin Health and Hygiene
I understand that grappling involves close physical contact and that communicable skin conditions
(ringworm, staph, MRSA, herpes gladiatorum, warts, impetigo) can spread on the mats. I agree to shower
before class when possible, wear clean training gear, keep my finger and toe nails trimmed, and refrain
from training if I have any open wounds, active skin infections, or contagious illnesses. I understand
that the Academy reserves the right to refuse participation if I have or appear to have a communicable
condition, and I will not hold the Academy liable for such refusal.
Injury Reporting
I agree to inform the instructor of any pre-existing injuries or medical conditions before training,
and to promptly report any injury sustained during training. I acknowledge that I am responsible for
my own medical care and insurance. I will seek medical clearance before returning to training after
any significant injury, illness, surgery, or concussion.
Photo, Video, and Likeness
I grant Not Bad BJJ permission to use photographs, video recordings, and my likeness taken during
training, competition, promotions, or other Academy events for promotional and educational purposes,
including on Academy social media accounts, the Academy website, and third-party media placements.
I can revoke this permission in writing at any time as to future uses.
Minor Participants
If the participant is under 18 years of age, this Waiver must be signed by a parent or legal guardian.
The parent or guardian acknowledges each risk on the minor's behalf, consents to the minor's
participation, and agrees to the terms of this Waiver as applied to the minor.
Membership, Freeze, and Cancellation
I acknowledge that monthly dues are non-refundable once billed. I may pause my membership (a "freeze")
for a documented injury, medical event, extended travel, or other qualifying circumstance by providing
written notice to Academy staff. I may cancel my membership at any time with written notice; cancellation
is effective at the end of the current billing period.
Code of Conduct
I agree to conduct myself with respect toward other members and Academy staff, to follow the directions
of instructors, to train with control and awareness for my partners, and to abstain from behavior that
is dangerous, disruptive, dishonest, or contrary to the values of the Academy. I understand that
violation of the Code of Conduct may result in suspension or termination of my membership without
refund.
Release of Liability
In consideration of being permitted to participate in training at Not Bad BJJ, I, on behalf of myself,
my heirs, executors, administrators, and assigns, do hereby release, waive, and discharge Not Bad BJJ,
its owners, instructors, members, employees, agents, and property owners from any and all claims,
demands, actions, causes of action, and liability arising from or related to my participation,
including claims of negligence, to the fullest extent permitted by Georgia law. This Waiver does not
release the Academy from claims of gross negligence, willful misconduct, or intentional harm.
Governing Law
This Waiver is governed by the laws of the State of Georgia. Any dispute arising from this Waiver or
from my participation at the Academy shall be resolved in the courts or arbitral forum of Cobb County,
Georgia.
Acknowledgment
I have read this Waiver in full. I understand its terms. I sign it freely and voluntarily.
